Mercy Hospice Team
Once a patient is referred for hospice care, the hospice professionals, along with the patient's physician, develop an individualized care plan to optimize comfort and the sense of personal control, addressing the needs and concerns of the patient and their family.
The Mercy Hospice Team meets regularly to discuss current cases, coordinate program components and ensure the quality and continuity of care.
Our expert, interdisciplinary hospice teams include:
- Patient's physician
- Hospice medical director
- Registered nurses
- Clinical social workers
- Chaplains
- Home health aides
- Volunteers
- Physical, occupational and speech therapists
- Nutritionists
- Pharmacy consultants
- Bereavement support for a year following the patient's death
